Dear Constantine,

I meet with Dr. Shapiro next Monday, July 7th for a 2nd opinion.  What questions should I ask him?  Also, I think you are aware I am triple negative.  Thank you so much for any help you can give me.  I want to make this appointment as beneficial as I can!  I feel so good that I have the opportunity to meet with him, even though I have already had my surgery, chemo and radiation and it is after the fact.

Thanks again!
Cheryl

No problem - I'll have some suggestions for you by the end of today at latest, probably in the evening (eastern time).

Here are my suggestions for the core ground to cover with Charles Shapiro in your upcoming consult:

  1. What is your impression of the primary regimen that was used to treat my triple negative disease?
  2. Now, after completion of this regimen, what is the current thinking as to any maintenance therapy?
  3. What would you advise as to the type, and frequency, of (1) radiological (MRI, PET/CT, sscans etc.) monitoring and (2) any biomarker or molecular marker monitoring in my case?
  4. Given the potential predilection of triple negative / basal for CNS metastases as recurrence, would special brain radiological, or any other, monitoring be advised?
  5. What other medical interventions are possible to lower my risk of recurrence?
  6. Would you advise that I be under the care of a breast specialist with some substantial triple negative experience, and who would you advise (given geographical limits)?
  7. (Optional Question)
What do you see as the most promising new developments in the treatment of triple negative disease?
[this question is more general and so you may want to ask it only if time and the spirit of the consultation suggest it to be appropriate]

Best of luck to you, and let me know if I can be of further assistance.
